Mission:

The mission of EDUCATION CONNECTION's Supported Living Services is to promote independence while providing long lasting personalized supports. We assist consumers in acquiring and maintaining living situations in the community that meet their goals and needs. We focus on community integration with the expectation that consumers will eventually have something to offer to the community in return.

Objectives:

• Protecting emotional well being
• Giving back to the community
• Building healthy reciprocal relationships

Accessing Services:

A referral from an individual, family member, guardian, or DMR case manager to EDUCATION CONNECTION.

Services Provided:

• Skills and needs assessment
• Community exploration
• Locating housing in desired areas
• Community acclimation
• Ongoing personalized supports; teach daily living skills, address medical issues and appointments, transportation, recreation coordination, budgeting and banking skills, family supports when needed.
• In home respite care
• Coordinate with other involved service providers
