Supported Living:

The mission of EDUCATION CONNECTION’s Supported Living Services is to promote independence while providing long-lasting personalized supports. We focus on community integration, with the expectation that consumers will have something to offer the community in return.

Promoting physical safety and health management
Protecting emotional well being
Giving back to the community
Building healthy reciprocal relationships

Supported Living Services provides the following:

Skills and needs assessment
Community exploration
Locating suitable housing
Community acclimation
In-home supports
Ongoing personalized supports:

teaching daily living skills
addressing medical issues
making appointments
arranging for transportation
coordinating recreational activities
developing budgeting and banking skills
organizing family supports when needed.

Assisting consumers and/or families with maintaining entitlements

Consumers are eligible to receive these services if referred to EDUCATION CONNECTION by a Department of Developmental Services (DDS) case manager, a consumer, a family member, or a guardian.
