Supported Living:

The mission of EDUCATION CONNECTION’s Supported Living Services is to promote independence while providing long-lasting personalized supports. We focus on community integration, with the expectation that consumers will have something to offer the community in return.

  • Promoting physical safety and health management
  • Protecting emotional well being
  • Giving back to the community
  • Building healthy reciprocal relationships

Supported Living Services provides the following:

  • Skills and needs assessment
  • Community exploration
  • Locating suitable housing
  • Community acclimation
  • In-home supports
  • Ongoing personalized supports:
    • teaching daily living skills
    • addressing medical issues
    • making appointments
    • arranging for transportation
    • coordinating recreational activities
    • developing budgeting and banking skills
    • organizing family supports when needed.
  • Assisting consumers and/or families with maintaining entitlements

Consumers are eligible to receive these services if referred to EDUCATION CONNECTION by a Department of Developmental Services (DDS) case manager, a consumer, a family member, or a guardian.
